Artists' Access to Healthcare (AAH) is a partnership of Springboard for the Arts, Neighborhood Involvement Program (N.I.P), People's Center Health Services, Doctor Sam
and the Twin Cities Musicians Union (TCMU) designed to help uninsured and underinsured artists receive low-cost medical care.
Update: Please note that AAH vouchers for Doctor Sam are on hiatus. During this time, Dr. Sam is working in earnest with a wonderful team of collaborators to improve
upon what is working about his office and to develop solutions for those areas which require them. It is difficult to put a timeline on this work, but please stay tuned to

How it works:
1) Three low-cost clinics in Minneapolis partner with Springboard to offer AAH vouchers. To get a voucher for a particular clinic, please fill out the cooresponding application below:

Please note you are eligible for one Doctor Sam, N.I.P. and People's Center Health Services voucher per year (3 vouchers total).
2) Within 48 hours (and usually the same day we receive your application), you can pick up your voucher at Springboard's office, or we will mail it to you.
3) Call and schedule your appointment at the clinic. For contact information and a list of each clinic's services and fee schedule, please click the links below:

4) At your appointment, present the AAH voucher and receive up to $40 off the cost of services and lab fees at the N.I.P. Community Clinic or People's Center Health Services,
or $100 off the cost of services, lab fees and membership at Doctor Sam. The voucher can only be used towards the cost of one visit. Costs over the voucher amount will be the
client's responsibility; however, clinics may be able to work with you on a flexible payment schedule. Members of the Twin Cities Musicians Union (TCMU) are also eligible for
a reimbursement voucher for follow-up care at N.I.P. to the extent of $75, when the Union member sends a reimbursement voucher and receipt for N.I.P. services to TCMU.
